Facility Coordinator

The overall purpose of this job is to coordinate the operations of the Group Home Facility, provide leadership and support to Youth Care Workers, and serve on-call for crisis and staffing.

Essential Functions:

Coordinates the operations of the Group Home Facility

  • Create a welcoming, homelike environment.
  • Provide daily essentials for the home. Such as groceries, cleaning supplies, and other items deemed necessary.
  • Coordinate the maintenance of the home (inside/outside) by contacting landlord or replacing items within monthly budget.
  • Complete monthly safety drills and complete reports in Kcare.
  • Completes monthly audits to ensure safe operation of the group home facility.
  • Comply with and enforce all requirements by OIG and CARF in regards to the physical plant.
  • Keep up to date with all needed documentation in the house (chore charts, daily logs, menus, etc.)
  • Maintenance of company vehicle and oversight of mileage log.
  • Maintain monthly budget.
  • Enter receipts to DDM within 24 hours of purchase.
  • Maintain and utilize petty cash properly.
  • Responsible for ensuring youth have adequate clothing.
  • Ensure allowance has been spent by or for youth.
  • Help create/purchase therapeutic activities and needed items for children.
  • Work in collaboration with the Scheduling Coordinator to ensure adequate staffing coverage of the group home.
  • Carry out one day of administrative duties per week.
  • Attend monthly supervision with Operations Manager.
  • Attend all scheduled meetings.

Provides leadership and support to Youth Care Workers

  • Assess and monitor the continued suitability and competence of Youth Care Workers and the Group Home environment ensuring a safe, healthy, nurturing and learning setting for youth is provided.
  • Provide shadow shifts to new employees.
  • Mentor Youth Care Workers and ensure that YCW’s fully understand the tasks, competencies to be achieved and the standards expected.
  • Provide mentorship of the Positive Behavioral Support Plan.
  • Address any issues and concerns of staff and communicate information to the Operations Managers.
  • Provide therapeutic intervention and support to children and youth in Key Assets Care per the treatment plan, 4 shifts per week.

Serves on-call for staffing and crisis support

  • Serve as on call for as needed in rotation.
  • Respond on-site to staffing and crisis situations when on-call.
  • Provide nightly check-in and report via email.

  • Attend training and development deemed necessary for this position.

Qualifications:

  • High school Diploma or GED
  • 21 years of age or older
  • Valid Driver’s License
