Key Assets Kentucky is seeking a licensed and experienced Nurse Care Coordinator to oversee the healthcare needs of youth in our therapeutic group homes. This leadership role blends hands-on care with oversight of medication management, wellness checks, and medical compliance – making a real difference in the lives of vulnerable youth.

What Youāll Do
- Oversee all medical and health-related services for youth in residential care
- Supervise and support the Nurse Aide and Certified Medical Assistant
- Schedule, track, and follow up on medical appointments and care plans
- Ensure medication compliance and documentation meet regulatory standards
- Conduct monthly wellness checks and ensure continuity of care
- Serve as a liaison with providers and train staff in CPR/First Aid and medication procedures
What Weāre Looking For
- Active RN or LPN license in Kentucky (required), BSN Preferred
- At least 5 years of nursing experience; youth or behavioral health preferred
- Excellent organization and communication skills
- Ability to manage priorities across multiple homes and lead a small team
