What You’ll Do
- Complete assessments and develop individualized service plans for each client
- Link families with services like healthcare, education, housing, and more
- Monitor and document client progress through face-to-face support and coordination
- Advocate for clients across service systems and assist with problem-solving
- Maintain detailed records to ensure compliance with state regulations
What We’re Looking For
- Bachelor’s degree in a human services field (required)
- Minimum of 1 year post-graduation experience in social services (or Master’s degree) (required)
- Knowledge of community resources and the ability to collaborate with providers
- Strong communication, organization, and time management skills
- Valid driver’s license and proof of auto insurance (required)
