What You’ll Do

  • Complete assessments and develop individualized service plans for each client
  • Link families with services like healthcare, education, housing, and more
  • Monitor and document client progress through face-to-face support and coordination
  • Advocate for clients across service systems and assist with problem-solving
  • Maintain detailed records to ensure compliance with state regulations

What We’re Looking For

  • Bachelor’s degree in a human services field (required)
  • Minimum of 1 year post-graduation experience in social services (or Master’s degree) (required)
  • Knowledge of community resources and the ability to collaborate with providers
  • Strong communication, organization, and time management skills
  • Valid driver’s license and proof of auto insurance (required)
