To provide high-quality support to the children in the CCC Program in direct relation to improving medication management and administration in order to promote the well-being and safety of the children in care.

Essential Functions:

  • The Nurse Care Coordinator acts as a liaison between patients and health care providers to ensure patients are receiving services.
  • Complete monthly health assessments of all youth in their homes.
  • Complete admission assessments and placement change assessments within 24 hours
  • Prepare for the biweekly clinic and assist the PCP and APRN during the clinic:
    • Prepare schedule and medication lists
    • Complete vital signs
    • Obtain labs as needed
    • Document and implement medication changes as needed
    • Run Kaspers as needed for APRN
  • Responsible for ensuring medication management procedures are adhered to and adjusted as needed for all 7 CCC Group Homes.
  • Maintain medical files, logs, and documentation related to the medical care of CCC youth.
  • Create and maintain the youth’s Medical Passport if one does not accompany the youth to placement.
  • Coordinate medical services for youth; communicate medical appointments to Operations Managers and the Scheduling Coordinator.
  • Communicate and collaborate with DCBS regarding medication changes, medication errors, and significant events.
  • Collaborate with schools regarding school hour medications, paperwork, and medical concerns.
  • Track medical appointments on an Excel spreadsheet to ensure compliance with DCBS regulations.
  • Coordinate and attend all medical appointments as required or deemed necessary.
  • Collaborate with CCC Leadership Team members on the medical needs of youth.
  • Complete MCO documentation and provide input for treatment plans.
  • Ensure EMR documentation is completed in a timely manner and according to set standards.
  • Ensure youth have the necessary medication in a timely manner:
    • Calling in medications;
    • Picking up medications from the pharmacy, and
    • Distributing medications to Group Homes.
  • Creation and maintenance of MARs (Medication Administration Records) for all youth in the CCC program.
  • Update medication lists on all youth to ensure medical providers have accurate information.
  • Complete medication counts of all CCC youth two times per month.
  • Create and post signs for all medication changes and medical alerts in all CCC group homes.
  • Conduct audits of group homes in regards to medication management as deemed necessary and complete monthly audits of medication timers.
  • Complete the monthly menu and disseminate it to the group home staff.
  • Conduct monthly training of Medication Administration Training (5 hours) in the evening.
  • Conduct monthly CPR training (5 hours) in the evening.
  • Attend all required agency meetings.
  • Attend all trainings and meetings in regards to the youth on the case load.
  • Serve after hours on-call per schedule (additional pay provided).

  • Attend training and development deemed necessary for this position.

Qualifications:

  • RN or greater clinical licensure. BSN is preferred.
  • A minimum of 5-7 years of experience is preferred.
  • Experience in psychiatric, residential, and/or adolescent care is strongly preferred.
  • Experience with psychotropic medications is strongly preferred.

Work Environment:

This job primarily operates from the Lexington Office. The Nurse Care Coordinator will be required to travel to the group home settings as deemed necessary in order to carry out all job duties effectively. Employee must be able to travel to our three group home locations: Lexington, Nicholasville, or Georgetown. A work cell phone and laptop will be issued for this role.
