Community Support Associate

The overall purpose of this job is to train, coach and support clients or their caregivers in the accomplishment of daily living skills.

Responsible To

Essential Functions:

The CSA operates under the supervision of the BHSO supervisor.

  • Train, coach and support clients and or caregivers in daily living skills
  • Assist clients and or caregivers with housekeeping, cooking, personal grooming.
  • Assist clients and or caregivers with accessing transportation.
  • Assist clients and or caregivers with budgeting, paying bills and maintaining an independent residence.
  • Assist clients in emergency situations to access financial, housing and other public services.
  • Assist clients in meeting their needs for food, shelter, and clothing.
  • Assist clients in accessing and utilizing community agencies and resources.
  • Assist clients in ongoing social, educational, vocational, recreational support and activities.
  • Assist clients in developing their objectives for their Person-Centered Plan (PCP)
  • Accompany clients to activities within the community.
  • Follow up with clients regarding health and service appointments.
  • Follow up with clients regarding completion of forms, returning forms or receipts and other similar activities.
  • Participate in Treatment Team Meetings
  • Participate in monitoring client and family satisfaction with support, services, and programs.
  • Maintain working knowledge of current trends and developments in field.
  • Serve as point of contact for crisis situations as needed.
  • Maintain and submit all required records, documents, and reports in a timely manner.
  • Create and maintain files for each client’s caseload.
  • Attend all required agency meetings.

Additional Duties:

  • Attend training and development deemed necessary for this position.
  • Ensure that Health and Safety is observed in the course of employment.
  • Ensure compliance with Department for Behavioral Health, Development, and Intellectual Disabilities and agency policies and procedures.
  • Ensure compliance with Equal Opportunities Policy and Procedure in all employment.
  • May be required to assume other duties as may be requested by the NCC that is commensurate with level of responsibility and skills.

Qualifications:

  • Must be 18 years or older.
  • Must have a High School Diploma or GED
  • Mus have one (1) year of full-time experience working with individuals who receive services for treatment of a 1) Behavioral health disorder or 2) co-occurring disorder and successfully complete within six (6) months of hire the department-led or approvedtraining program described in Section 3 of 908 KAR 2:250.
  • Must have a valid Kentucky driver’s license and proof of auto insurance.
